Several methods of non-surgical skin tightening are currently available. Each technology varies considerably in its depth of penetration.

The skin is made up of two layers, the visible outer layer called the epidermis and the deeper layer called the dermis. The main structural support of the dermis is provided by the protein called collagen.

Beneath the dermis lies the hypodermis. It consists of fat cells, collagen, elastin and fibrous bands that anchor the skin via connective tissue to deeper structures such as muscle and bone.

The thickness of skin varies from 0.5mm thick on the eyelids to 4.0mm thick on the heels of the feet.

The choice of which skin tightening technology to use is usually determined by the thickness of the loose or lax skin requiring treatment.

How does the treatment work?

Skin remodeling is a biophysical phenomenon that occurs at cellular and molecular levels via the intervention of optical or thermal energy.

Skin tightening technologies exert their biological effect on the skin through two main mechanisms:

  • Heat-induced collage shrinkage and micro-thermal injury
  • Dermis regeneration, repair (wound healing) and remodeling.

Thermal cleavage of hydrogen bond cross links in the collagen triple helix is thought to result in contraction of the structure by up to 1/3 of its original length.

Cellular contraction involves the initiation of an inflammatory/wound healing sequence that is perpetuated over several weeks. Contraction of skin is achieved through fibroblastic multiplication and contraction with the deposition of a static supporting matrix of nascent scar collagen. The cellular contraction process is initiated by de-granulation of mast cells that release histamine initiating the inflammatory wound healing response.

Following cellular contraction, collagen is laid down as a static supporting matrix in the soft tissue structure. The deposition and subsequent remodeling of this nascent scar matrix provides the means to alter the consistency and geometry of the soft tissue for aesthetic purposes.

An unprecedented demand for non-surgical skin tightening has resulted in the development of several technologies aimed at treating lax or loose skin.

How does the treatment work?

Near infrared technologies operate in the 780nm-950nm wavelength range. The 780nm-950nm optical window allows deep penetration (up to 2.5mm) and deep dermal heating due to heat conduction with low melanin and water absorption in the epidermal and upper dermal layers. NIR tightens the skin by two separate processes.

Firstly it has an immediate effect by tightening the supportive framework of the skin (by contraction of collagen fibres and tissue retraction) aiding remodelling and rejuvenation.

Secondly, heating the dermis stimulates the body to produce collagen, boosting a healing response. This acts as a new deep foundation for the skin producing further tightening that becomes evident over several months.

The combination of these two processes hence provide immediate skin tightening and further long-term tightening effects.

Can all areas of the body be treated and does skin colour make a difference?

Any patient with loose or lax skin can have the treatment and skin colour makes little difference to the effectiveness of the procedure. It should be noted that NIR technology cannot be applied to patients with severe heart disease, diseases of connective and muscular tissue or pregnant and breast feeding women.

How long does it take and does it hurt?

Depending on the size of the area being treated the procedure can take as little as 15 minutes up to a maximum of several hours for the treatment of extensive areas. The patient experiences a gentle heating sensation during the procedure. There is no pain and because the surface layer of the skin is not damaged there is no prolonged healing time and normal activities can be resumed immediately.

Are there any side effects?

NIR procedures have an excellent safety record. Redness (erythema) of the treatment area is usually noted during the procedure and resolves within 30 minutes to 24 hours following treatment. Rarely swelling and blistering in the treated area may occur. These side effects usually settle within a few days.

How many treatments are needed?

Typically a series of 6 treatments at 4-6 weekly intervals are recommended for optimal results. Methods to reduce further collagen breakdown includes smoking cessation and the application of a moisturiser with an SPF30 sun block. These will enhance the longevity of the treatment. It must be remembered that the skin is constantly ageing so it is envisaged that further treatment in the future may well be desirable.

It should be noted that Radio Frequency cannot be applied to patients with pacemakers, metal plates or pins in the treatment area, severe heart disease, diseases of connective and muscular tissue or pregnant and breast feeding women.
